Code of Conduct, Rules & Regulations Packet


UNIFORM/EQUIPMENT RESPONSIBILITY

Leyden Township Boys Football(L.T.B.F) provides equipment and uniform for all participants.  It is the responsibility of the parent/guardian to keep these uniforms clean & in good condition and return them at the end of the season.

The parent/guardian will supply the participant with the following:

 

Football: Football shoes, athletic supporter, rib vest (optional), and neck roll (optional).

Cheer: L.T.B.F. will provide complete uniform.

 

DOCUMENTATION REQUIREMENTS

L.T.B.F. requires the following documents before assignment to a team, receiving any equipment or attending any practices.  Failure to provide these documents will result in mandatory drop.

1.  The registration form signed by parent or legal guardian.

2.  Code of Conduct, Rules & Regulations "received" form signed by parent or legal guardian.

3.  A copy of certified Birth Certificate.

4.  Proof of sports physical signed by a qualified physician and dated on or after January 1st, of the current year.   This is due before Aug. 1st.

5.  A copy of the last report card from the previous school year.  This is also due by Aug. 1st.

DROP POLICY 

L.T.B.F. has a voluntary and mandatory drop policy:

VOLUNTARY:  If a child chooses to drop from the program, no fees will be refunded, unless you drop less than 10 days from after you register your child.  All uniforms and equipment must be returned immediately.

MANDATORY:  If a child is dropped from the program for any of the following reasons, no fees will be refunded:  1) Failure to pay registration fees; 2) Failure to provide necessary documents; 3) Failure to turn in fundraising money; 4) The child is a disciplinary problem; 5) Failure to follow any AYF rules.

FUNDRAISERS

The registration fees alone are not adequate to fund this program.  Therefore, it is necessary for each family, with a registered child, to participate in our fundraisers.  Failure to do so will void the child's registration.

BOARD MEETINGS

Our board meetings are held at Grant Park on the 3rd Tuesday of every month @ 6:30 pm.  If you are interested in observing a meeting, please confirm date and time by calling Bill Otte @ 708-562-8323.

TEAMS

Football

Placement of your child well be the decision of the L.T.B.F. Board, based on age, weight, and ability.

Cheer

Placement of your child will be the decision of the Cheer Coordinators, based on age, weight, and ability.

PRACTICE  INFORMATION

Both football & cheerleading teams start practice Aug. 1st.  10 hours per week before the new school year starts and 6 hours per week during school at Grant Park in Northlake.

Note: Cheer - Dates and locations subject to change.

Family vacations should be scheduled around the Leyden Bear season which runs from August thru December.
